Appointments

All patients are seen by appointment only. Referral by your Pediatrician is recommended. You will be offered the earliest appointment. If you are unable to keep your appointment, please contact us 24 hours in advance or you may be charged $25.00 for missed appointment. If you arrive later than 15 minutes for your appointment, you may be asked to reschedule. For any changes call us at (727) 498-8994.

What to Bring:

» Please bring any insurance with you. Your appointment may be rescheduled if you do not have proper authorization to be seen.

» Please bring your current insurance card and photo ID.

» Please bring a list of the Prescriptions your child is currently taking.

» In cases of divorce, both parents and/or guardians of the patient are welcome to the appointment with proper authorization.

» Case workers and/or guardians must have court documents showing that they are in their care.
